ASSESSMENT OF POSTOPERATIVE PAIN AFTER PROTAPER NEXT VERSUS WAVE ONE NITI SYSTEMS IN PATIENTS WITH IRREVERSIBLE PULPITIS (A RANDOMIZED CONTROLLED TRIAL)

ABSTRACT
This Study Was Conducted To Evaluate Postoperative Pain After Root Canal Preparation With ProTaper Next And WaveOne NiTi Systems. Fifty Two Patients With Mandibular Permanent Premolars And Molars, Showing Signs/symptoms Of Symptomatic Irreversible Pulpitis Were Selected From The Outpatient Clinic Of Endodontic Department, Cairo University. An Approval From The Ethic Committee And An Informed Consent From The Patients Were Taken. Patients Were Randomly Divided Into Two Groups; Comparator Group: Root Canal Instrumented With WaveOne Instruments (n=26), And Intervention Group: Root Canal Instrumented With Protaper Next (PTN) Instruments (n=26). Postoperative Pain Was Recorded Using Numerical Rating Scale (NRS) At 6, 12, 24, 48 Hours Intervals. Results Showed That A Significantly Lower Levels Of Postoperative Pain In The PTN Group Compared To WaveOne Group At 6 And12 Hours. No Significant Difference Was Found Between Both Groups At 24 And 48 Hours. In Conclusion, PTN Rotary System Was Able To Minimize The Risk Of Postoperative Pain Intensity, As Compared To WaveOne Reciprocating System In The First 24 Hour.

IMMUNOHISTOCHMEICAL EXPRESSION OF CLAUDIN 1& 3 IN RADICULAR CYST, ODONTOGENIC KERATOCYST, UNICYSTIC AMELOBLASTOMA AND CONVENTIONAL AMELOBLASTOMA

ABSTRACT
Odontogenic Cysts And Tumors Have A Wide Spectrum Of Histopathologic And Clinical Characteristics, Ranging From Benign Non-invasive To Locally Invasive Or Malignant Lesions. However, Little Is Elucidated About Their Pathogenesis. The Aim Of Study Was To Evaluate The Immunohistochemical Expression Of Claudin-1 And Claudin-3 In The Most Common Odontogenic Lesions. Comparing The Expression Levels Of These Two Markers Among Those Lesions And Finally, Correlation Of The Obtained Results With Their Biological Behaviors Were Also Performed. Forty Archival Cases Of Radicular Cyst, Odontogenic Keratocyst, Unicystic Ameloblastoma And Ameloblastoma (10 Cases Each) Were Stained With Hematoxylin And Eosin For Histological Examination. Streptavidin-biotin Method Was Used For Immunohistochemical Staining By Anti Claudin-1 And Anti Claudin-3 Antibodies. Our Results Revealed That The Expression Level Of Both Markers Was Gradually Decreased Starting From Radicular Cyst To Odontogenic Keratocyst, Unicystic Ameloblastoma Until Reaching Its Lowest Value In Ameloblastoma. The Statistical Analysis Of The Current Work Revealed A Highly Significant Difference In Area Percentage Of Both Markers Among The Examined Groups. In Addition, The Area Percentage Of Claudin-1immunoexpression Was Much Higher Than That Of Claudin-3, And The Correlation Between Both Markers Was Positive. In Conclusion, Claudins-1 And -3 Could Be Useful Prognostic Biomarkers In Odontogenic Lesions. However, Claudin-1 Is More Specific For Odontogenic Lesions Than Claudin-3. Besides, They Could Aid In Differentiation Of The Neoplastic From Non-neoplastic Odontogenic Lesions And Could Be Auxiliary Indicators Of Aggressiveness And Invasion Potentials.

ABSTRACT
The Aim Of The Study : Evaluation Of The Osseointegration Of Direct Laser Metal Forming Immediate Implants For Replacement Of Upper Anterior Teeth.

Materials and Methods: Twelve Patients With A Need For Upper Anterior Teeth Replacement Was Randomly Assigned To Be Treated With Direct Laser Metal Forming (DLMF) Immediate Implant (Tixos). Implants Were Loaded After 6 Months Of Placement. The Follow Up Was Clinical Evaluation Included (Bleeding On Probing, Pocket Depth, Plaque Index ,Gingival Index), A Comparison Table Between One Week After Loading And 6 Months After Loading. Radiographically A Cone Beam C.T Was Taken After 1 Week Of Placement And 6 Months Of Placement Were Evaluated For Change In Relative Bone Density Adjacent To The Implants. Results : All Implants Showed Initial Primary Stability And Good Signs Of Osseointegration With Absence Of Any Signs Of Failure. The Clinical Evaluation Was Enhanced After 6 Months Of Loading Comparing To 1 Week After Loading. The Mean Relative Bone Density Increased After 6 Months Of Implant Placement Comparing To One Week After Placement. Conclusion : All Twelve Implants Have Been Successfully Present In The Bone And Completely Osseointegrated By Complete Bone Formation And Density Around All Implants That Can Function With Good Esthetics.

The Aim Of This Study Was To Assess The Incidence Of Fistula In Isolated Cleft Palate Patients Repaired By Furlow Double-opposing Z-plasty Versus Two-flap Palatoplasty At Different Follow-up Periods. One Case Of The Furlow Group Showed Fistula Formation. However, Three Cases Had Nasal Regurgitation Only In The Two Flap Group. Amount Of Intra-operative Blood Loss Was Significant In The Furlow Group; It Was Mostly Related To The Extent Of The Surgical Field And Time Of Surgery.

ABSTRACT

Objectives: Microhardness Evaluation Of Initial Enamel Lesions By Bioactive Glass Air-abrasion Before NovaMin Application. Methods: 40 Extracted Human Molars Were Sectioned Mesiodistally And Were Embedded In Resin Molds. Specimens Were Assigned To 4 Groups According To The Remineralization Protocol: G1- (artificial Saliva), G2- (air-abrasion), G3- (NovaMin), G4- (air-abrasion + NovaMin). Specimens Were Immersed In A Daily Renewed Demineralizing Solution To Create Artificial White Spot Lesions. Specimens Were Stored In A Daily Renewed Artificial Saliva. Microhardness Was Assessed Using Vicker�s Hardness Number (VHN) At Starting Point, After Demineralization, After 24 Hours And One Month Remineralization.

Results: The Group Of Air-abrasion That Received NovaMin For One Month Showed The Highest Statistically Significant VHN, With No Statistically Significant Difference Between The VHN Values Of This Group After Remineralization And At Starting Point.

Conclusions: Air-abrasion Of Enamel With Bioactive Glass Did Not Play A Role To Improve Remineralization.

ABSTRACT
Temporomandibular Joint TMJ Disorders TMDs Is A Umbrella Term For A Variety Of Conditions Of The Joint And Mastication Muscles. TMJ Arthrocentesis And Intra-articular Injection Are Commonly Known And Used As Minimally Invasive Surgical Treatment Of Tmj Disc Displacement With Reduction. Aim Of The Study: To Compare Between Arthrocentesis Followed By Intra Articular Injection With Plasma Rich In Growth Factors PRGF Versus Arthrocentesis Alone. Patients And Methods: Sixteen Patients Diagnosed With Anterior Disc Displacement (ADD) Were Divided Into; Intervention And Comparative Group. Clinical Outcome Parameters Were Assessed At Baseline And At 6, 12, 18 And 30 Weeks Postoperatively.

Results: Both Groups Showed Significant Improvement Of Pain And Mouth Opening, The Evaluated Parameters Showed Insignificant Differences Between The Two Groups Throughout The Study Intervals With The Exception That The PRGF Group Showed Statistically Significant Pain Reduction And Mouth Opening Improvement In 30 And 6 Weeks Postoperative Respectively (p = 0.01 , 0.03) Conclusion: Both TMJ Arthrocentesis With Intra-articular Injection Of PRGF Or Arthrocentesis Only Was Proved To Reduce Symptoms Of Pain And Limited Mouth Opening And To Maintain Improvements Over Time. PRGF Injection After Arthrocentesis Revealed Some Superiority In Pain Reduction And Mouth Opening Over Arthrocentesis In 30, 6 Weeks Interval Respectively.

ABSTRACT
The Aim Of The Study Was To Evaluate Symmetry Between Reconstructed Zygomatic Complexes And The Normal Sides Using CT. Between 2013 And 2015, 7 Patients Suffering From Unilateral Orbitozygomatic Defects Were Selected From The Out-patient Clinic Of The Oral And Maxillofacial Surgery Department, Faculty Of Dentistry, Cairo University And Computed Tomography (CT) Scan Of The Facial Bone Was Requested. The Image Acquisition Parameters Were; Axial Cuts, Minimal Slice Thickness, Minimal Slice Distance And Of A Bony Window. The Patients Underwent Reconstruction Of The Deficient Zygomas Using CAD/CAM Generated Implants. One Month Post-operative CT Was Done And Measurements Of Symmetry Were Performed Using Surgical Simulation Software (Mimics, Materialise, Leuven, Belgium). Points Representing The Maximum Lateral Facial Prominence Were Marked On The Corresponding 2D CT Images Of Both Normal And Reconstructed Sides; The Distances Between These Points And The Mid Sagittal Plan Were Measured On Both Sides. Results Measuring From The Mid-sagittal Plane, The Difference Between The Maximum Projections Of The Reconstructed And The Unaffected Sides Ranged From 0.3 Mm To 6.25 Mm With A Mean Of 2.35 Mm Conclusion Evaluation Of Symmetry Between Reconstructed Zygomatic Complex And The Unaffected Side Of The Face Can Be Simply Performed Using Computer Simulation Software And That CAD/CAM Generated Implants Can Provide A High Degree Of Accuracy In Maxillofacial Reconstruction.

THE EFFECT OF BITE RAISING ON ROOT INTEGRITY. SCANNING ELECTRON MICROSCOPIC AND LIGHT MICROSCOPIC STUDY

ABSTRACT


Introduction: Raising The Bite Is A Common Method In Orthodontics To Facilitate Several Treatment Procedures; However, This Method May Transmit Excessive Occlusal Forces Through The Periodontal Ligament. This Study Was Carried Out To Evaluate The Integrity Of The Root After Bite Raising To 3 Mm. Methods: Eighty Maxillary And Mandibular First Premolars Of 20 Patients Comprised The Sample For The Present Study. The Right Side Of Each Patient Was Selected As The Control While The Contralateral Side Was The Experimental. By The End Of The Experimental Period, One Month For Group I And Two Months For Group II , The First Premolars Were Extracted. Half Of The Samples Were Prepared For Scanning Electron Microscopic Examination, And The Others Were Prepared For Light Microscopic Examination.

Results: Root Resorption Began As A Superficial Multiple, Discrete, Small Pits That Merged And Coalesced To Form Wide Shallow Bays In Both Control Subgroups And Experimental Group I. In Experimental Group II, The Resorption Was Extended Deeply With Dentin Involvement And Undermining Resorption Was Observed. With Light Microscopic Examination, Mononucleated And Multinucleated Cells Were Noted In Relation To The Resorptive Areas. There Were Significant Differences In The Surface Area Of Root Resorption Between The Experimental And Control Subgroups, Between Experimental Subgroup I And Subgroup II. Apical Third Was The Most Common Affected One At Both Control With Significant Differences. Repair Of Resorped Areas Occurred In 80% Of The Control, 57% Of The Experimental Group I And Increased To 83% For The Control And 71% For The Experimental Group II. Conclusion: Root Resorption Occurred Concurrent With Bite Raising By 3 Mm For One And Two Months And Its Severity Increased With Time. Repair Of The Resorbed Area Started Early During Force Application And Increased With Time.

Objective: This Randomized Clinical Trial Aimed To Compare Between Platform Switched Implants And Platform Matched Implants Regarding Change In Marginal Bone Level Up To 9 Month After Final Prosthetic Reconstruction Using Cone Beam Computed Tomography (CBCT) And Periotest.

Materials and Methods: The Study Included A Total Of 26 Implants In 12 Patients, Inserted And Positioned At The Crestal Bone Level, And Randomly Assigned Into Two Groups Either Platform-matched (control) Or A Platform-switched (mismatch 0.35-0.6 Mm) Implant Abutment Connection (test). Outcome Measures Were Patient?s Satisfaction, Implant Mobility, And Radiographic Crestal Bone-level Changes. These Parameters Were Assessed Preoperatively, 3 Months After Implant Placement And 9 Months After Placing The Implant Crown.

Results: For All The Measured Outcomes, There Was No Statistically Significant Difference Between The Two Groups. However After 9 Months, Platform-switched Group Showed Statistically Significantly Lower Mean Percentage Of Crestal Bone Loss Than Platform-matched Group.

Conclusions: Within The Limitation Of The Current Study, The Results Suggested That There Was No Difference Between Both Groups And Platform Diameter Does Not Have A True Impact Upon Hard Tissue Response Around Dental Implants.

Background Cleft Lip And Palate Is One Of Most Common Congenital Craniofacial Deformities. A Congenital Cleft Lip Deformity Has Significant Physical And Psychological. Successful Repair Of Cleft Lip Deformity Is A Challenging As Well As Rewarding Task. No One Technique Of Cleft Lip Repair Consistently Produces Ideal Aesthetic And Functional Results. Objectives To Assess The Vermillion Symmetry And Scar Quality In Surgical Management Of Patients With Unilateral Cleft Lip Using Modified Millard?s Technique Versus Fisher?s Technique. Patient And Methods This Study Was Conducted On A Group Of Twenty Patients, Younger Than Six Months Of Age, Who Were Born With Unilateral Cleft Lip. Patients Were Selected From The Outpatient Clinic Of Department Of Plastic Pediatric Surgery, The Specialized Pediatric Hospital, Faculty Of Medicine, Cairo University. All The Patients Were Evaluated Preoperatively, On 7th Postoperative Day , Three Months And Six Months Follow Up. Comparison Between The Two Techniques Was Made Keeping In Mind The Aesthetic And Functional Aspects Of The Repair. Results There Were Small Difference Between The Two Techniques In Notching Of The Vermillion Border And Symmetry Of The Free Vermilion Border Was Similar With Both Repair Techniques. There Are No Scars In The Nares, Allowing For An Improved Nasal Form With The Fisher Technique Compared To The Rotation Advancement Flap. In The Other Areas Of Evaluation Of The Closure, There Were No Significant Differences. Conclusion No One Technique Of Cleft Lip Repair Consistently Produces Ideal Aesthetic And Functional Results.
