METFORMIN GEL AS AN ADJUNCT IN TREATMENT OF CHRONIC PERIODONTITIS IN SMOKERS CLINICAL AND IMMUNOLOGIC STUDY

ABSTRACT
Metformin Is One Of The Most Commonly Used Anti-hyperglycemic Drugs To Treat Type II Diabetes Mellitus, It Was Shown That Metformin Has A Positive Effect On Bone Turn Over Represented In Decreasing Osteoclastic Activity. The Current Study Aimed To Investigate The Effect Of The Adjunct Use Of Locally Applied 1% Metformin Gel In The Periodontal Pocket, On The Outcome Of Nonsurgical Conventional Treatment Of Moderate Chronic Periodontitis In Smokers. Methods: Twenty Smoker Patients Diagnosed As Moderate To Severe Chronic Periodontitis Were Selected And Divided Into Two Groups The 1st Is The Study Group That Was Treated By Conventional Treatment (scaling And Root Planing) + Local Delivery Of 1% Metformin Gel (MF) While The 2nd Is The Control Group That Was Treated By Conventional Treatment + Local Application Of Placebo Gel. The Evaluation Of Both Groups Based On Evaluation Of Clinical Parameters (periodontal Probing Depth (PPD), Clinical Attachment Level (CAL), Gingival Index (GI) And Plaque Index (PI) In Addition To Biologic And Immunologic Parameters (levels Of Alkaline Phosphatase Enzyme (APE) And Interleukin 1 Beta (IL1?) In Gingival Crevicular Fluid (GCF). The Measurements Recorded At Base Line, After 1 Month, 3 Months And 6 Months Of The Treatment. Results : PPD Reduction And CAL Gain Was Higher In The 1st Group (MF Group) Than The 2nd Group (placebo Group), Also The GI Enhancement Was Higher In MF Group Than Placebo Group. There Was A Decrease In Levels Of IL1? In Both Groups But At 3 And 6 Months More Reduction Was Seen In MF Group, The Levels Of APE Was Decreased In Both Groups After 1 Month But The Study Group Showed Higher APE Levels At 3 And 6 Months When Compared To 1st Month Levels But Still Less Than The Base Line Level. Conclusion: 1) According To Clinical Results, The Adjunct Use Of Locally Applied 1%MF Gel To Conventional Non-surgical Treatment Has A Beneficial Positive Effect On The Clinical Outcome Of The Treatment. 2) According To Biologic And Immunologic Results The Adjunct Use Of Locally Applied 1%MF Gel To Conventional Non-surgical Treatment Has An Anti-inflammatory Effect And Bone Formation Effect.

MICROHARDNESS OF ARTIFICIAL WHITE SPOT LESIONS TREATED WITH RESIN INFILTRANT AND AMORPHOUS CALCIUM PHOSPHATE

ABSTRACT


Purpose: The Purpose Of This Study Was To Evaluate The Effect Of Three Different Surface Treatments On Surface Microhardness Of Artificial White Spot Lesions At Different Times Of Measurement.

Materials and Methods: A Total Of 65 Anterior Bovine Teeth Were Randomly Assigned To 12 Groups (n=5) According To The Two Experimental Factors Being Investigated (surface Treatment And Time Of Measurement). Labial Enamel Surfaces Were Wet Ground Using SiC Paper Grits # 180, 600, And 1200, Then, Acrylic Base Was Poured. Artificial White Spot Lesions Were Created In The Outlined Window Of Sound Enamel Measuring 6mm?6mm Using Demineralizing Solution. Different Surface Treatments Were Performed According To The Tested Times Of Measurement Which Are Immediately, After 2 Weeks, After 4 Weeks, And After 8 Weeks On Artificial White Spot Lesions Specimens; G1: Artificial Saliva, G2: Resin Infiltrant And G3: CPP-ACP. Representative Specimen From Each Group Was Evaluated Using Environmental Scanning Electron Microscope. Then, Specimens Were Submitted To Vickers Microhardness Test After Surface Treatment. ANOVA Was Used To Analyze The Data.

Results: Two-Way ANOVA Showed A Significant Effect Of Surface Treatment And Time Of Measurement On Surface Microhardness Of Artificial White Spot Lesion. Meanwhile, The Interaction Between Surface Treatment And Time Of Measurement Had No Significant Effect On Surface Microhardnes (p > 0.001). Conclusion: Resin Infiltrant Was More Effective In Improving Surface Microhardness Of Enamel Immediately. Meanwhile, CPP-ACP Seems To Only Impart An Increase In Microhardness After 4 Weeks.

Background: Mandibular Implant-supported Overdentures Solved The Problems Of Retention, Stability And Patient Comfort. In These Cases The Implants May Be Inserted With Angulations That Affect The Choice Of Attachments Used. This Study Aimed To Evaluate Effect Of Implant Angulations On Retention Of Two Stud Attachments For Implant Supported Mandibular Overdenture.

Materials and Methods: Two Implants Were Fixed At Canine Region Bilaterally In A Heat Cured Acrylic Resin Model At (0, 5, 20)?. Ball And Socket Attachment As Well As Locator Were Tested At The Aforementioned Angles. Retention Force Was Measured After Applying Insertion And Removal Cycles Simulating 24 Months Using Universal Testing Machine. The Summed Retentive Values Were Compared Using Paired T Test.

Results: Both Attachments Showed Retention Loss From Initial Values Till End Of Simulation Period, At (0, 5)? Ball And Socket Attachment Showed Higher Mean Retentive Force Values Than Locator, While At 20? Locator Attachment Showed Higher Mean Retentive Force Values. Conclusion: Under The Limitations Of Current Study, It Can Be Concluded That Both Attachments Shows Retention Loss After Simulated Period Of Usage. At (0, 5)? Angles, Ball And Socket Attachment Is More Retentive Than Locator, While At 200 Angle Locator Attachment Is More Retentive Than Ball And Socket.

Background: Dental Caries Is A Chronic, Multifactorial Disease, With Limited Epidemiological Data Available For The Egyptian Population.?

Aim: This Study Aimed To Measure The Prevalence Of Dental Caries Among A Group Of Egyptian Preschool Children In El Suez City. Methods: A Cross Sectional Study Was Carried Out In El Suez City. 1200 Children Aged 3 - 5 Years? Old Attending Private And Public Kindergarten Were Included In The Study. Children Were Examined For Dental Caries Using Dmft Index Of Primary Teeth. Comparison Of Numerical Variables Between The Study Groups Was Done Using Student T Test For Independent Samples. For Comparing Categorical Data, Chi-square (c2) Test Was Performed.

Results: The Prevalence Of Dental Caries Among A Group Of Preschool Children In El Suez City Was (63. 3%). The Mean Dmft Was (2.36 ? 0.99) In Males And (3.02 ? 0.55) In Females. Conclusion: Dental Caries Prevalence Is High Among Preschool Children In El Suez City.

ABSTRACT
This Research Work Was Executed To Assess And Relate The Cyto-toxic Outcome Of A Recently Introduced Tricalcium Silicate Bioceramic Sealer (BioRoot? RCS) In Parallel With A Salicylate-resin Based Sealer (MTA-Fillapex). Sealer Discs With 5mm In Diameter And 2mm In Height Were Prepared Using Teflon Molds. The Effects Of Sealer Extracts On Cell Vitality And Proliferation Were Evaluated Using A Methyl-thiazoldiphenyl-tetrazolium (MTT) Assay. The Experimentations Were Executed Using MRC-5 Cells At 37 ?C In 100% Humidified Atmosphere Containing 5% CO2. Cells Were Subjected To Different Concentrations Of The Tested Sealers? Extracts And Cytotoxicity Was Assessed At 24 Hrs. And 72 Hrs. Relative To Controls. Data Were Explored For Normality Using Kolmogorov-Smirnov Test. For Parametric Data, Independent T Test Was Used To Compare All Groups. Paired-t Test Was Utilized For Comparing The Same Group At Different Observation Times. The Significance Level Was Set At P ? 0.05. BioRoot-RCS Was Significantly Less Cytotoxic Than MTA-Fillapex With A Proliferative Time Dependent Profile.

Background: Quality Of Care And Its Assessment Is An Extensive Task Of Health Care Providers All Over The World. One Of The Major Ways To Estimate The Quality Of Health Services Is The Evaluation Of Patients? Satisfaction.

Aim: Determine The Level Of Patients? Satisfaction With The Quality Of Dental Services Provided At Tanta Dental College. Design: The Study Was Cross Sectional Observational Design. A Random Sample Of 900 Participants Was Selected From Outpatient Clinics Of Faculty Of Dentistry, Tanta University. Data Were Collected Using Validated Arabic Version Of Dental Satisfaction Questionnaire Which Included 19 Items Divided Into Four Domains Including Patient?personnel Interaction, Technical Competency, System/administrative Efficiency And Clinic Setup/environment. Results Were Statistically Analyzed Using Descriptive Statistics And Chi-square Tests (p< 0.05).

Results: Results Showed That The Highest Rate Of The Participants Satisfaction Was Recorded From Patient-dentist Interaction, Followed By Technical Competency And Administrative Efficiency. On The Other Hand, The Least Satisfaction Was Reported From Clinic Setup Environment. There Was No Significant Association Between Socio-demographic Characteristics Of The Study Sample Including Age, Gender, Nationality, Occupation, Income, Marital Status, Educational Level And Overall Satisfaction.

Conclusions: Patients Were Highly Satisfied With The Patient?dentist Interaction, Technical Competency And Administrative Efficiency And Least Satisfied With Clinic Setup Environment. Socio-demographic Characteristics Did Not Influence The Level Of Patients? Satisfaction.

This Clinical Study Was Conducted To Evaluate And Compare The Post-operative Pain After The Using Of Two Different Irrigating Techniques : Activation Of Irrigation Using Diode Laser And Conventinal Needle With Endvented Navitip Immediate Postoperatively And 4, 12, 24, 48, 72 Hours And 7 Days Utilizing A Numerical Rating Scale (NRS). 38 Patients With Acute Pulpitis With Apical Periodontitis, Root Canals Were Prepared Using NiTi ProTaper Universal Rotary System Then Randomized Into Two Equal Groups According To The Technique Used For Irrigation Group (A) Activation Of Irrigation Using Diode Laser And Group (B) Conventional Syringe With Endvented NaviTip? Irrigating Tip, The Needle Of Irrigation Were Penetrated 2 Mm Shorter Than The Working Length. The Trial Design Of This Study Is A Parallel Randomized Controlled Trial. All Demographic Data, Clinical And Radiographic Findings And Modified NRS Scores Obtained From Patients Were Statistically Analyzed. Results Showed That There Was No Statistically Difference Between The Two Groups Regarding The Demographic Data, Prevalence Of Pre-operative Pain, After 4 Hours, 12 Hours, 24 Hours, 48 Hours And 7 Days , While In Both Groups There Was A Statistically Significant Decrease In Pain Intensity Preoperatively Compared With All Other Time Periods.

The Aim Of This Study Was To Compare Fracture Resistance Of Roots Obturated With Totalfill BC Sealer Using Two Obturation Techniques.

Materials and Methods: Thirty Single-canal Extracted Mandibular Premolars Were Selected. The Crowns Were Removed And The Roots Were Prepared With Fanta Rotary System To Size 40 Taper 4 And Obturated With Totalfill Bioceramic Sealer. Roots Were Divided Into Two Groups According To Obturation Technique (n?= 15). Group I: Lateral Compaction Technique Was Used And Group II: Single Cone Technique Was Used. All Specimens Were Tested In A Universal Testing Machine As The Force At The Time Of Fracture Was Recorded In Newtons. Data Were Analyzed Using The One-way Analysis Of Variance.

Results: Showed That Group II Has Significantly Higher Values (419.5?34.9N) Than Group 1 (292.6?3.8N). Conclusion: It Can Be Concluded That Single Cone Obturation With Bioceramic Sealer Has The Ability To Increase The Fracture Resistance Of Root Canals After Instrumentation, And Thus Reinforcing Endodontically Treated Teeth.

ABSTRACT
Purpose Compare Retention And Wear Characteristics Of Locator And OT Equator Attachments Used In Two Implant Retained Mandibular Overdenture.

Materials and Methods: Fourteen Ready Mandibular Epoxy Resin Models Were Used. Two Implant Fixtures With 4 Mm Diameter And 11mm Length Were Installed In The Canine Area. Group (I) Received Locator Abutments With White Nylon Inserts While Group (II) Received OT Equator Attachments With Clear Nylon Inserts. The Final Constructed Mandibular Overdentures Were Subjected To 240,000 Cycle Of Bi-axial Chewing Simulating Cycles And 1080 Manual Insertion-removal Cycles. The Retention Of The Samples Were Measured Using Universal Testing Machine At The Beginning Of The Study And After 3,6,9,12 Months Of Functional Simulation. Wear Features In The Nylon Inserts Were Also Observed By USB Digital Microscope With Built In Camera At The Beginning Of The Study And At 3,6,9,12 Months. Results The Locator Attachments Showed 31.89% Loss Of Initial Retention After One Year Of Simulated Function Compared To 46.44% Loss Of OT Equator Retention. Notable Wear And Surface Alterations Were Visible Under Photomicrograph On Nylon Inserts Of Both Attachments. However, Both Systems Nevertheless Fulfilled The Minimum Retention Requirements Put Forward By Other Authors During Simulation Of One Year Functional Life. Conclusion Statistically Significant Differences In Retention Develop Between The Two Systems, With OT Equator Undergoing A Greater Loss Of Retention Than Locator.

ABSTRACT


Aim: The Aim Of The Study Is To Compare Between The Effect Of Plasma Rich In Growth Factors (PRGF) And Leukocyte And Platete Rich In Fibin (L- PRF) Mixed With Bone Graft On The Primary Stability Of Implants Placed With Trans-alveolar Sinus Lifting In The Posterior Edentulous Maxilla. Materials And Method: Fourteen Partially Edentulous Patients With Maxillary Kennedy Class II Mod. 1 Were Selected And Divided Into Two Groups. Closed Sinus Lifting Technique And Implant Placement Were Done For All Patients In Both Groups. In Group A Bone Graft Was Mixed With PRGF While In Group B It Was Mixed With L-PRF. Primary Implant Stability Was Measured Using Resonance Frequency Analysis By Osstell At Insertion, After 4 Weeks And After 12 Weeks.

Results: Group B Showed Lower Decrease In Stability After 4 Weeks While From 4 To 12 Weeks Showed Higher Increase In Implant Stability Than Group A. Regarding The Difference Between Both Groups Throughout The Studied Period, Group A Showed Greater Decrease In Implant Stability Than Group B And It Was Statistically Significant. Conclusion: Using L-PRF During Transalveolar Sinus Augmentation Achieved Better Implant Stability Than PRGF.
