EVALUATION OF POST-OPERATIVE PAIN AFTER IRRIGATION USING END-VENTED NAVITIP TIPS VERSUS SIDE-VENTED NAVITIP TIPS IN TEETH WITH IRREVERSIBLE PULPITIS: A RANDOMIZED CLINICAL TRIAL: PART V

ABSTRACT

Objectives: The Aim Of This Prospective Randomized Clinical Trial Was To Compare The Degree Of Post-operative Pain And Analgesic Intake At 0, 4, 12, 24, 48, 72-h And 7 Days After The Use Of Two Different Irrigation Needles. Subjects And Methods: Thirty Eight Participants Diagnosed With Symptomatic Or Asymptomatic Irreversible Pulpitis In Mandibular Posterior Teeth Received Single-visit Root Canal Treatment And Were Divided Randomly Into Two Groups According To The Type Of Needle Used During Irrigation Either NaviTip 29-gauge, 27 Mm With End-Vented Needle (EVN) Or NaviTip 31-gauge, 27 Mm With Double Side-port Irrigator Tip (SVN). Post-operative Pain Was Measured Using Numeric Rating Scale (NRS) Pre-operatively, Immediate Post-operatively And At 4, 12, 24, 48, 72-h And 7 Days. Placebo And Analgesic Intake Was Recorded At The Different Time Intervals.

Results: There Was A Statistically Significance Difference In Post-operative Pain Between Both Groups At 12, 24, 48 And 72-h Post-operatively. A Significant Increase In Pain Level Was Recorded After 4-h Compared To The Immediate Post-operative In The End-vented Needle Group, Followed By A Gradual Decrease In Pain Levels Until Pain Disappeared At 7 Days With No Difference In Medication (placebo And/or Analgesic) Intake Post-operatively Between Both Groups.

Aim : To Evaluate The Effect Of TotalFill Bioceramic Sealer Versus AH Plus Resin Sealer On Cell Viability And To Compare The Effect Of The TotalFill Bioceramic Sealer Versus AH Plus Resin Sealer On Diameter Of Zones Of Bacterial Inhibition On Enterococcus Faecalis, Porphyromonas Gingivalis And Porphyromonas Endodontalis. Methods: Cytotoxicity Was Analyzed On Baby Hamster Kidney�fibroblasts� (�BHK) Submitted To Cell Culture Medium Conditioned By Sealers Using The MTT Reduction Assay. Cells Grown On Fresh Medium Served As Control. Cell Viabilities Were Measured At 24, 48 And 72 Hours. The Percentage Of Viable Cells In Each Well Was Calculated. The Antibacterial Activity Was Analyzed On An Enterococcus Faecalis Strain (ATCC 29212), Porphyromonas Gingivalis And Porphyromonas Endodontalis Using Agar Diffusion Test(ADT). The Inhibition Zones In ADT Were Measured After 48 Hours.

Results: Cultures Submitted To TotalFill BC Sealer Had A Significantly Higher Number Of Viable Cells (p<0.05) Than AH Plus Sealer. TotalFill BC Sealer Exhibited Significantly Smaller Inhibition Zones Than AH Plus With (p<0.05) On Enterococcus Faecalis, While It Showed Statistically Significant Larger Inhibition On P.endodontalis And On P.gingivalis Than AH Plus . Conclusion: The Bioceramic-based Sealer (TotalFill) Has Higher Biocompatibility Than The Resin Sealer (AH Plus). TotalFill Sealer Has Lower Antimicrobial Effect On E.faecalis And Higher Antimicrobial Effect On P.endodontalis And P.gingivalis In Comparison To AH Plus Sealer.

The Present Study Was Conducted To Compare Between The Amount Of Produced Damage From Diode Laser Incisions With Different Wavelengths And/or Powers On The Collateral Tissue In The Buccal Mucosa Of New Zealand White Rabbits. Fourty Five Adult Rabbits (each 2000 Grams Wt.) Were Utilized For The Study And Were Equally Sorted Into 9 Groups. Group I (control Group With No Intervention) Represented The Normal Healthy Rabbits. Then 1 Cm Incision Was Performed In The Buccal Mucosa Of Rabbits Using Diode Laser In All Experimental Groups As Follows: Group II: 980 Nm With Power 1.5 Watts, Group III: 980 Nm With Power 2.5 Watts, Group IV: 810 Nm With Power 1.5 Watts, Group V: 810 Nm With Power 2.5 Watts, Group VI: 980 Nm With Power 1.5 Watts, Group VII: 980 Nm With Power 2.5 Watts, Group VIII: 810 Nm With Power 1.5 Watts, Group IX: 810 Nm With Power 2.5 Watts. Histological Assessment Of The Buccal Mucosa Specimens In Vicinity Of The Incisions Was Performed Immediately And After 3days Post Incision. The Histological Results Included Carbonization, Cytoplasmic Vacuolization, Inflammatory Cell Infiltration And Collagen Hyalinization In Epithelium And Lamina Propria. Conclusion: Marked Histopathological Changes Were Observed Among Both Wavelengths Upon Application Of Different Powers. 810 Nm Wavelength Produced Lesser Inflammatory Changes When Compared To 980 That Were Only Significant With Powers; 1.5 Watts At Day Zero And 2.5 Watts At Day 3.

ABSTRACT


Aim: To Assess The Effectiveness Of Using Fluoride-containing Mouth Rinse In Preserving The Integrity Of Hard Tooth Structures In High Soda Drinks Consumers.

Materials and Methods: 48 Adult Volunteers Were Recruited To Participate In This Study. The Volunteers Willing To Participate In The Trial Were Asked To Complete A Self-administrated Questionnaire. They Were Asked To Recall The Frequency And Amount Of Acidic Foods And Drinks Consumed Per Day. The Participants Were Divided Into Two Groups According To The Type Of Interventions Used; Fluoride-containing Mouth Rinse And Placebo. Each Group Was Evaluated At Four Different Evaluation Periods; At Baseline, After 3 Months, After 6 Months And After 12 Months. The Intervention Group Was Given Fluoride-containing Mouth Rinse While The Control Group Was Given Sodium Chloride Intravenous Infusion To Use As Placebo Mouth Rinse. Both Groups Were Instructed To Use Fluoride-containing Tooth Paste. Assessment Of Erosive Tooth Wear Was Done Using The Erosion Index Proposed By Lussi, While Assessment Of Dental Caries Was Done Using The International Caries Detection And Assessment System (ICDAS). Statistical Analysis Was Done Using Kolmogorov-Smirnov, Shapiro-Wilk Tests, Mann Whitney And Wilson Signed Rank Test.

Results: For Erosive Tooth Wear: At Baseline, The Intervention Group (0.61�0.50), The Control Group (0.49�0.51). At 3 Months, The Intervention Group (0.61�0.50), The Control Group (0.55�0.54). At 6 Months, The Intervention Group (0.61�0.57), The Control Group (0.67�0.64). At 12 Months, The Intervention Group (0.72�0.66), The Control Group (0.76�0.68). For Dental Caries: At Baseline, The Intervention Group (2.15�1.35), The Control Group (2.11�1.24). At 3 Months, The Intervention Group (2.09�1.35), The Control Group (2.15�1.24). At 6 Months, The Intervention Group (2.14�1.47), The Control Group (2.30�1.39). At 12 Months, The Intervention Group (2.26�1.59, The Control Group (2.57�1.50). Conclusion: The Use Of Fluoride-containing Mouth Rinse Could Not Preserve The Integrity Of Hard Tooth Structures In High Soda Drinks Consumers.

ABSTRACT
Statement Of The Problem: Conservative Approach In Modern Dentistry Aims To Restore The Missing Part Of Tooth Structure With A Material That Has Improved Mechanical And Esthetic Properties. One Of Recently Introduced Material For Construction Of Fixed Restoration Is Vita Suprinity. It Is A Glass Ceramic Reinforced With A 10% Zirconia. It Was Assumed That This Modification Improved The Mechanical Properties Of Fixed Restorations. It Is Not Yet Known Whether This Modification In The Microstructure Enabled The Material To Be Used Successfully With Conservative Tooth Preparation Using Feather Edge Finish Line Without Compromising Restoration Strength. Aim Of This Study: Evaluation Of The Fracture Resistance Of Zirconia Reinforced Lithium Silicate (vita Suprinity) Single Crowns With Two Margin Geometries (feather Edge And Chamfer Edge) Under Thermal And Cyclic Loading.

Materials and Methods: A Total Of Twenty Six Samples (epoxy Resin Dies) Were Used In The Present Study (n=26) That Were Divided Into Two Groups (n=13 For Each Group). These Epoxy Dies Were Duplicated From Two Stainless Steel Dies, One Of These Dies Was Prepared With Chamfer Finish Line And Other Was Prepared With Feather Finish Line. Then Scanning Of Stainless Steel Dies Was Carried Out For Milling Of Vita Suprinity Crowns With Two Finish Line Configurations. After That Crowns Were Cemented To Epoxy Dies And Then Exposed To Thermo-mechanical Aging That Was Repeated For 37,500 Cycles Which Simulate 3 Months Of Clinical Usage. Then Fracture Resistance Test Was Carried Out Using A Computer Controlled Universal Testing Machine With A Loadcell Of 5 KN And Data Were Recorded Using Computer Software.

Results: It Was Found That, The Chamfer Finish Line Group Recorded Statistically Non-significant Higher Fracture Resistance Than The Feather Edge Group At (p Value>0.05).

Conclusions: Feather Edge Margin Geometry Is A Promising Conservative Alternative To Chamfer Margin When It Is Used For Construction Of Zirconia Reinforced Lithium Silicate (vita Suprinity) Crowns Without Adverse Effect On The Fracture Resistance.

Aim Of Study: To Validate The Reliability Of Cone Beam Computed Tomography (CBCT) Corrected HU Using The Correlation Equation Of The CBCT Gray Values, With True Hounsfield Units (HU) Derived From Multislice Computed Tomography (MSCT) As A Gold Standard. Methods: The Study Was Performed On Human Dry Mandibles Acquired From The Department Of Anatomy, Faculty Of Medicine, Cairo University. Soft Tissue Simulation In The Form Of Full Mandibular Coverage Was Done By Adapting Eight Sheets Of Softened Pink Wax Approximately 12mm Thickness. Mandibles Were Scanned By Both Cone Beam Computed Tomography (CBCT) And Multislice CT Followed By Importing The Dicom Files From Both CBCT And CT Into Outsider Third Party Software (On-Demand Software) Which Empowers Our Measurements.

Results: Pearson Correlation Showed A Weak Correlation Between Voxel Gray Values In CBCT With HU In MSCT In All Selected Areas. Conclusion: The Corrected HU Derived From Voxel Gray Values Obtained From CBCT Revealed A Weak To Fair Correlation With HU Obtained From MSCT. CBCT Readings On Measuring Bone Density Were Not Reliable.

Statement Of Problem: Maxillary Anterior Tooth Loss May Cause Esthetic And Functional Problems. Successful Treatment For These Situations May Necessitate A Minimally Invasive Technique To Preserve Sound Remaining Abutment Tooth Structure Objective: The Aim Of This In-vitro Investigation Was The Establishment Of Three Different Proposed Designs Of A Conservative Partial Fixed Restorative Ceramic Approach For Restoring Missing Maxillary Lateral Incisor, Aiming To Assess Their Mechanical Performance Using Two Different Ceramic Materials Employing CAD-CAM Technology Material & Methods: The Same 3 Designs Were Prepared As Previously Mentioned Typodont Model With Typodont Resin Teeth Representing Missing Maxillary Lateral Incisor Case Was Used. Maxillary Central Incisor And Maxillary Canine Were Prepared According To The Following Three Designs: First Design (Group I): Minimal Invasive Preparation Design Second Design (Group II): Palatal Preparation With Proximal Half Grooves. Third Design (Group III): Palatal Preparation With Proximal Half Grooves And Cingulum Indentation.) Optical Impressions Were Taken For The Preparations To Obtain A Model For Each Preparation Of The Three Groups , And All- Ceramic Fixed Partial Dentures (30 IPS E-max CAD And 30 Incoris TZI Ceramic Material) Were Fabricated In Accordance With The Different Designs. Duplicate The Three Master Models For The Purpose Of Production Of Epoxy Models That Were Used As Substrates For Cementation Of The Constructed Resin Bonded Bridge Samples During Testing Procedures. Bridges Were Cemented To The Epoxy Models . Thermo-mechanical Aging Test Was Conducted Using Chewing Simulator Accompanied With Thermo-cyclic Mode.The Test Was Repeated 120,000 Times To Clinically Simulate The 6 Months Chewing Condition, Accompanying Thermocycling According To Previous Studies Temperature Changes Between 5oC And 55oC Using A Frequency Of 3 Hz .Then Fracture Test Was Done By Compressive Mode Of Load Applied At The 135o Angle (through Housing The Sample In Specially Designed 45o Angle Jig) Using A Metallic Rod With Round Tip (3.8 Mm Diameter) Attached To The Upper Movable Compartment Of Testing Machine Traveling At Cross-head Speed Of 1mm/min. Statistical Analysis Was Performed After Collecting All Data.

Results: Regarding The Fracture Resistance Test, For Zirconia Bridges ; It Was Found That Group II Recorded Statistically Significant Highest Fracture Resistance Mean Value (612.72 � 63.8 N) Followed By Group III With Mean Values (530.34 � 27.2 N) While Group I Recorded The Lowest Fracture Resistance Mean Value (500.44 � 79.6 N). These Results Showed Non-significant Difference Between Group III And Group I. For E.max Bridges ; It Was Found That Group III Recorded Statistically Significant Highest Fracture Resistance Mean Value (420.3� 33.4 N) Followed By Group II With Mean Values (391.03 � 12.1 N) While Group I Recorded The Lowest Significant Fracture Resistance Mean Value (321.57 � 16.3 N). Regardless To Preparation Designs, Totally It Was Found That Zirconia Bridges Recorded Statistically Significant Higher Fracture Resistance Mean Value (547.83 � 43.26 N) Than E.max Bridges Mean �SD Values (377.63 � 37.38 N). Irrespective Of Ceramic, Totally It Was Found That Group II Recorded Statistically Significant Highest Fracture Resistance Mean Value (501.88 � 37.9 N) Followed By Group III With Mean Values�SDs (475.32 �30.3 N) While Group I Recorded The Lowest Fracture Resistance Mean Value (411.01 � 47.9 N) . Results Showed Statistically Non-significant Mean Vales Between Group III And Group II.

Conclusions: Within The Limitations Of The Present Study, The Following Conclusions Could Be Drawn: All-ceramic Resin Bonded Fixed Partial Denture (RBFPD) Restoring The Maxillary Lateral Incisor Could Withstand Successfully The Maximum Loads To Which The Anterior Area Is Subjected. Incoris TZI And IPS E-max Blocks Using Cerec-Inlab System Could Be Used Successfully For Construction Of Resin Bonded Fixed Partial Dentures Incoris TZI Resin Bonded Bridges Had Superior Fracture Resistance Than IPS E-max CAD Resin Bonded Bridges. The Design Has Significant Effect On Fracture Resistance, Adding Retentive Means Increases The Fracture Resistance Of Both Materials.

Purpose: The Present Study Was Conducted To Evaluate The Amount Of Bone Height Gain, Density Values And Implant Stability After Maxillary Sinus Floor Elevation Using Graftless Tenting Technique Or The Use Of Hydroxyapatite Nanoparticles Bone Substitute For Augmentation With Simultaneous Implant Placement.

Materials and Methods: A Total Of 20 Sinuses With A Residual Alveolar Bone Height Ranging From 4-6mm Were Divided Into Two Groups And Underwent Sinus Augmentation Using Nano Hydroxyapatite Bone Substitute Material And The Graftless Tenting Technique With Simultaneous Implant Placement. Computed Tomography CT Scans And ISQ Measurements Were Conducted To Evaluate Bone Quality, Quantity And Implant Stability.

Results: Radiographic Analysis Revealed That The Mean Bone Height Gain Of The Nano Group Was 7� 0.8mm Compared To 5�1.5mm In The Tent Group. The Mean Bone Density Of The Nano Group Was 548�25 Compared To 420�23 In The Tent Group. The Mean ISQ Value After 6 Months In The Nano Group Was 78�5 Compared To 77�5 In The Tent Group. Conclusion: Nano Hydroxyapatite Bone Graft Offered Superior Results In Terms Of The Bone Height Gain And The Relative Bone Density As Compared To Graftless Tenting Technique. However, Both Techniques Showed Accepted Results Regarding Implant Stability.

Purpose: The Aim Of This In-vitro Study Was To Compare The Retention Of Esthetic Equipoise Clasp Versus Conventional (Aker) Clasps In Bilateral Bounded Removable Partial Denture Cases.

Materials and Methods: Two Standard Experimental Models Of Maxillary Edentulous Case (Kennedy Class III With Modification I) Missing Right And Left Second Premolar And First Molar Were Fabricated To Be Used As A Master Model To Simulate The Oral Cavity. Eleven Cobalt-chromium (Co-Cr) Alloy Frameworks Of Each Clasp Design Engaged 0.01 Inch Undercut For All Clasps, Mesiobuccal Of First Premolar And Distobuccal Of Second Molar On Both Sides For Aker And On Disto-proximal At The Axial Of The Buccal Premolar And On Mesio-proximal At The Axial Of The Buccal Molar On Both Sides Of Equipoise Clasp. The Loop Was Centralized And Soldered Into The Major Connector. Digital Force Gauge Was Used For Both Groups To Measure The Retention. The Pulling Force Was Gradually Applied Until The Framework Dislodged.

Results: The Highest Mean Value Of Retention Was Found In (Aker Clasp) (94.85 � 7.17) While, The Lowest Mean Value Of Retention Was Found In (Equipoise Clasp) (61.06 � 3.52). Conclusion: It Could Be Concluded That The Mean Retentive Force Required To Remove The Equipoise Clasp Was Found To Be Lower Than The Force Required Removing Aker Clasps.

ABSTRACT
Background: Quercetin Is An Effective Cytotoxic Anticancer Agent And Has Been Used For The Treatment Of Variety Of Malignancies. The Aim Of The Current Study Is To Investigate The Influence Of Quercetin On Squamous Cell Carcinoma Cell Line (HEP-2) After 24 Hours. Material & Methods: Treatment Of Cell Line (HEp-2) By Different Concentrations Of Quercetin Was Done To Assess The Viability Of The Treated Cells To Determine The Percentages Of Viable Cells That Was Monitored Using MTT Assay.

Results: The Mean Viability Percentage Of Quercetin Treated Cells (in Relation To Control Cells) Increased As Quercetin Concentration Decreased From 640 MM To 5mM With An IC50 Of 200 MM. Regarding The Cytotoxic Effect Of Quercetin And Related Cell Cycle Profile It Was Noticed That Cell Distribution Showed A Variable Percent Of Arrest At Different Phase Of Cell Division Profile, Where There Was Non Significant Difference Of Arrested Cells Of Quercetin Pre And Post Treatment Compared With Its Value In Non Treated G0-G1 Phase Control Cells(P>0.05) And S Phase As Well. While There Was A Significant Elevated Arrest Of Treated Cells During The G2-M Phase (P<0.05) And The Significant Difference Of Cell Arrest At G2-M Phase Was Type Of Treatment Related . Conclusion: From The Results Of The Current Study, We Found That Quercetin Has Cytotoxic Effect On Squamous Cell Carcinoma, (HEp-2) Cell Line, It Also Induced An Effect On The Cell Cycle Distribution, And Quercetin Has Effect On (HEp-2) Cell Line Resulting In Apoptosis And Necrosis.
