ACCURACY OF STITCHED SMALL FOV CBCT IN THE ASSESSMENT OF MANDIBULAR LINEAR MEASUREMENTS: A DIAGNOSTIC ACCURACY STUDY

ABSTRACT
The First Implementation Of Dental CBCT Stitch Module Carried Out In 2010 Paved The Way Towards Wider Range Of Clinical Applications For Small Field Of View (SFOV) And Increased The Affordability Of CBCT Systems. Limited Diagnostic Studies Were Carried Out To Assess The Accuracy Of The Stitch Module. This Study Aimed To Assess The Diagnostic Accuracy Of Stitched Small Field Of View (SSFOV) CBCT In Mandibular Linear Measurements Of Dry Human Mandibles.

Materials and Methods: Fifteen Mandibles Included In The Study Were Marked At 18 Anatomic Sites And Were Identified By Gutta Percha Rodes. Twenty Six Linear Measurements Were Performed Directly Using Digital Caliper For Each Mandible (gold Stander). The Mandibles Were Immersed Under Tap Water For Soft Tissue Simulation. Planmeca Promax 3D MID CBCT Machine Was Used To Scan The Mandibles Using 50 X 80 Mm FOV And 200 �m Voxel Resolution. Each Mandible Was Scanned 4 Times Sequentially To Cover The Entire Mandible. Stitching Was Conducted To Combine The Scanned Volumes Sequentially To Generate A Full Composite Image Of The Entire Mandible. Planmeca Romexis Viewer 3.5.1R, 3D Software Was Used To Record The Same 26 Linear Measurements. Statistical Analysis: Quantitative Variables Were Described. Normality Hypothesis Of All Quantitative Variables Were Checked For Further Choice Of Appropriate Parametric And Non-parametric Tests. For Inter- And Intra-observer Reliability, Interclass Correlation Coefficients (ICC) Including 95% Confidence Limits Was Used. The Significance Level Was Set At P � 0.05.

Results: As The Data Showed Normal Distribution, Independent Sample T Test Was Used For Comparing The Direct And The CBCT Linear Measurements. In All Of The Measurements, No Statistically Significance Difference Was Found Between The Direct Linear Measurements And CBCT Linear Measurements. Excellent Inter-observer And Intra-observer Reliability Was Found. Conclusion: Within The Limitations Of This Study, It Can Be Concluded That CBCT Stitch Module Using SSFOV Is An Accurate Tool To Generate A Larger Composite Image Of The Entire Mandible.

THE EFFECT OF IBUPROFEN, PARACETAMOL VERSUS PLACEBO ON PAIN DURING LOCAL ANESTHETIC INJECTION AND FOLLOWING DENTAL EXTRACTION IN PRIMARY MOLARS: A RANDOMIZED CLINICAL TRIAL

ABSTRACT
Objective: This Study Was Conducted To Assess Effectiveness Of Ibuprofen And Paracetamol Versus Placebo On Pain Perceptions During Local Anesthetic Injection And Following Dental Extraction Of Lower Primary Molars In Children. Subjects And Methods: A Randomized Controlled Clinical Trial Was Performed On 156 Apparently Healthy Egyptian Children Who Were Enrolled From The Outpatient Clinic In Pediatric Dentistry And Dental Public Health Department- Faculty Of Dentistry, Cairo University- Egypt. The Participants Were Divided Into Three Groups (52 Patients In Each Group). Each Group Received Specific Type Of Drug 30 Minutes Before Local Anesthetic Administration. The First Group Received Ibuprofen, The Second One Received Paracetamol And The Third Group Received Placebo. Pain Scores During The Injection Of Local Anesthesia And Following Dental Extraction Were Recorded Using The Facial Image Scale (Five-Face Scale). Pain Scores Were Recorded At The 1st, 2nd, 3rd, 4th, 5th, 6th And 24th Hours Following Dental Extraction From Parents By Telephone.

Results: The Use Of Pre-emptive Analgesia As Ibuprofen And Paracetamol Decreased The Pain Scores Significantly Compared To Placebo During Local Anesthetic Administration. Additionally, At The Third, Fourth , Fifth, Sixth And 24th Hours Following The Dental Extractions, The Results Showed Statistically Significant Differences Between The Three Groups With (p�0.001). Conclusion: The Use Of Pre-emptive Analgesia As Ibuprofen And Paracetamol Was Effective In Reducing Pain During Local Anesthetic Administration. Also, Both Ibuprofen And Paracetamol Were Potent And Effective In Reducing Pain Following Dental Extractions After Third, Fourth, Fifth, Sixth And 24th Hours, And There Was No Difference Between Them.

Statement Of Problem: Cementation Is A Critical Step That Determines The Success And Longevity Of Indirect Restorations. Cement Dissolution At The Marginal Interface Is One Of The Causes That Leads To Micro Leakage And Diminishes Bond Strength Of The Indirect Restoration Which Leads Eventually To Its Failure. New Bio-active Cement Was Introduced To Overcome That Disadvantage By Re-mineralizing Of The Disintegrated Joint. Due To The Lack Of Data About The Bioactive Cement, It Required Investigation To Assess Its Shear Bond Strength. Aim Of The Study: Is To Evaluate Shear Bond Strength Of Activa Bio-active And Self-adhesive Resin Cement When Bonded To Enamel And Zirconia. Methodology: 20 Extracted Human Mandibular Molars Were Prepared To Produce Flat Enamel Surface. Enamel Samples Were Divided Randomly Into Two Groups, Wet And Dry Where (n=10). Each Subgroup Was Bonded Using Either Self-adhesive Resin Cement As Control Group Or Bioactive Cement (n=5) To Zirconia. Shear Bond Strength Was Examined After Thermo-cycling Using Universal Testing Machine. Mann-Whitney U Test Was Used To Compare Between Wet And Dry Conditions And The Effect Of Different Cement Types. The Significance Level Was Set At P � 0.05.

Results: Either With Wet Or Dry Enamel There Was No Statistically Significant Difference. Both Cements Showed No Significant Difference When Bonded To Wet Or Dry Enamel. Conclusion: Neither The Different Humid Conditions Nor The Different Cement Types Demonstrated Significant Impact On The Shear Bond Strength Values.

ABSTRACT


Aim: The Aim Of This Study Was To Assess The Short-term Efficacy Of The Lingual Ring Splint In The Management Of Anterior Disc Displacement With Reduction For The Relief Of Pain And Improving Mandibular Movements. Patients And Methods: Ten Female Patients Diagnosed With DDWR Were Included In This Study. All Patients Received The Lingual Ring Device With Specific Instructions Regarding The Time Of Using It And Exercises. Follow Up Was Made After 1 Week, 1, 2, And 3 Months To Test Pain Score And Mandibular Range Of Motion.

Results: Patients Age Ranged From 19 To 32 Years Old. Pain Score Decreased Significantly After 1 Week, And Again After 1-3 Months. All Mandibular Movements Showed A Statistically Significant Increase In Range After 1 Week, And Again After 1-3 Months. Conclusion: The Lingual Ring Splint Is An Effective Therapy For The Management Of Patients With DDWR. Significant Improvement In Pain Score And Mouth Movements Should Be Noted Within 3 Months Follow Up If The Device Was Used With A Regimen Of This Study. Adenoid Cystic Carcinoma (ACC) Is The Third Most Common Malignant Salivary Gland Tumor, Cancer Stem Cells (CSCs) Are Defined Cells Within A Tumor That Possess The Capacity To Self-renew, CD133 (prominin-1) Is An Important Marker To Identify And Isolate CSCs. A Total Of 50 Archival Paraffin-embedded Specimens (10 Normal Salivary Gland Tissues, 10 Tubular, 10 Trabecular, 10 Cribriform And 10 Solid Cases) Were Collected. Sections Were Stained Immunohistochemically By CD133 Antibody And The Area Percent For Its Expression Was Evaluated By The Image Analyzer Computer System, It Was Found ThatCD133 Immunoexpressionhad High Significant Difference In Area Percent Values Between The Control Group And The Different Patterns Of ACC. CD133 Expression Significantly Increased In The Solid Pattern Compared To The Other Patterns. No Significant Difference Was Observed Between The Tubular And The Trabecular Patterns. In Conclusion, CD133 Was Detected In ACC And Its Expression Was Related To Poor Biological Behavior. It Could Be A Promising Target As Regards Anti-CSC Therapy. ACCURACY OF DENTAL IMPLANT PLACEMENT USING KEYLESS SLEEVELESS STEREOLITHOGRAPHIC SURGICAL STENTS

ABSTRACT

Objectives: Evaluation Of The Accuracy Of Dental Implant Placement Using Keyless Sleeveless Stererlithographic Surgical Stent. Methodology: The Study Was Designed As A Single Armed Clinical Trial. Ten Endosseous Tapered Implants Were Placed Using Keyless, Sleeveless Stereolithographic Surgical Stents. Postoperative Cone Beam Computed Tomography Was Taken And Superimposed On The Preoperative Virtual Plan To Assess Angular, Horizontal And Vertical Deviations.

Results: Results Showed That Implants Placed With Keyless Sleeveless Surgical Guide Had A Range Of Horizontal Deviation At The Coronal Side (C) Of The Implant Of 0.7-1.9 Mm With An Average Of 1.16 � 0.4 Mm. Horizontal Deviations At The Apex Of The Implants (Ap) Had A Range Of 0.2-1.46 With An Average Of 0.82 � 0.43 Mm. Vertical Deviations Of The Implants (V) Had A Range Of 0.1- 1.1mm With An Average Of 0.562� 0.34 Mm. Angular Deviations Between The Long Axis Of The Actual Implants To The Virtual Implants (A) Had A Range Of 0.51- 6.52 Degrees With An Average Of 2.7� 1.95 Degrees.

EVALUATION OF FLEXURAL STRENGTH OF MONOLITHIC ZIRCONIA AND ZIRCONIA CORE AFTER HYDROTHERMAL AGING (IN-VITRO STUDY)

ABSTRACT
Statement Of Problem: Concerns Has Been Raised With Regard To Low Temperature Degradation (LTD) Of Yttria Stabilized Tetragonal Zirconia Polycrystal (Y-TZP) For Monolithic Zirconia And Core Zirconia. However, Studies On Effect Of Low Temperature Degradation On Mechanical Characteristics Of Zirconia Were Insufficient. Aim Of Study: The Aim Of The Study Was To Assess The Effect Of Low Temperature Degradation (LTD) On Flexural Strength Of Monolithic Zirconia And Zirconia Core Using Hydrothermal Aging In Autoclave Condition For 8 Hours (relevant To Around 30 Years Of Clinical Service).

Materials and Methods: 32 Zirconia Discs Were Cut From 2 Zirconia Blocks(monolithic Zirconia And Zirconia Core) With Isomet -1000, Then Were Sintered According To Manufacturer Instructions To Dimensions (12mm Diameter And 1.2 Mm Thickness). Each Group Of Zirconia Was Subdivided According To Hydrothermal Aging Into Aged Subgroups And Non-aged Subgroups. Artificial Aging Was Done In Autoclave Condition At (134�C, 0.2 MPa) For 8 Hours. Non Aged Subgroups Represents Control. X-ray Diffraction (XRD) And Scanning Electron Microscope (SEM) Were Used To Analyze The Microstructure Of Both Y-TZP Types Before And After Aging. All Samples Were Subjected To Biaxial Flexural Strength Test With Ball On Ring In Universal Testing Machine. Data From Biaxial Test Were Analyzed Using Graph Pad Instat (Graph Pad, Inc.) Software.

Results: Zirconia Core Group Recorded Statistically Significant Higher Biaxial Flexure Strength Mean Value (1192.76�105.47 MPa) Than ZrO2 Monolithic Group (1033.13�136.68 MPa) Before Aging. After Aging Zirconia Core Group Also Recorded Statistically Significant Higher Biaxial Flexure Strength Mean Value (1305.52�159.67 MPa) Than Zirconia Monolithic Group (1084.55�91.43 MPa).XRD Analysis Showed Increase Of Monoclinic Content In Both Types Of Zirconia Surfaces After Aging. Conclusion: Hydrothermal Aging Induced Tetragonal To Monoclinic Transformation And Modified The Microstructure Of All Zirconia Types. Moreover, Hydrothermal Aging Didn?t Significantly Affect The Flexural Strength Of Monolithic And Core Y-TZP After 8 Hours Of Aging Within The Limitations Of This Study. Also, Both Monolithic Zirconia And Core Zirconia Can Resist Low Temperature Degradation Up To 24-32 Years Of Clinical Service. Clinical Implications: Based On The Results Of This In Vitro Study, Clinicians May Consider Leaving Zirconia Core Without Veneering Exposed In The Oral Environment In Areas Adjacent To The Soft Tissue Such As Implant Abutments And Substructures And On The Intaglio Surface Of Partial Fixed Dental Prosthesis Connectors (which Are Subjected To Tensile Stresses) Without Being Concerned About Any Significant Deterioration Of The Material And Subsequent Catastrophic Failure.

Objectives: The Aim Of The Present Study Is To Introduce A Novel Modality For Estimation Of The Anti-neoplastic Efficacy Of Chemotherapeutic Agents And To Compare This Technique With The DNA Fragmentation Assay. Methods: Squamous Cell Carcinoma Cell Line HEp-2 Was Used In The Present Study. The Cytotoxic Drug Used In This Study Was Sesamol. Microscopic Slides Of Each Drug Concentration At Definite Post-treatment Durations Were Photomicrographed And Analyzed Using Image Analysis Software For The Estimation Of Nuclear Area Factor. Data Were Then Statistically Analyzed. To Assess Whether The Morphological Changes Induced By The Drug Depended On Activation Of The Apoptotic Pathway, It Was Investigated Whether It Could Induce Internucleosomal DNA Fragmentation.

Results: The Data Recorded Revealed A Decrease In The Mean Values Of NAF Of HEp-2 Cells Treated With Different Concentrations Of Sesamol Over Different Durations When Compared To Control Cells. DNA Fragmentation Was Not Observed In The Control Groups Over Different Durations Of Incubation. The Study Of Internucleosomal DNA Fragmentation In HEp-2 Cells Treated With Different Concentrations Of Sesamol Over Different Durations Revealed Absence Of DNA Laddering That Indicated DNA Fragmentation With 6.25 ?M Sesamol Concentration After 24 Hours Of Incubation. Conclusion: The Present Data Suggested That Estimation Of Nuclear Area Factor Is A Sensitive Predictor Of Early Apoptotic Effect Of Anti-cancer Therapy. DNA Fragmentation Assay Is A Useful Tool For Detection Of Late Apoptotic Changes That Discriminates Apoptotic Effects Of Anti-cancer Drugs From Necrotic Ones.

Introduction: Effect Of Extraction On The Orthodontic Practice Was Claimed To Cause Decrease In The Airway Diameter And Affect The Hyoid Bone Position. Other Researchers Contradicted This Finding. The Aim Of The Study Was To Systematically Review The Literature To Resolve This Conflict.

Materials and Methods: The Study Included The Clinical Trials, Cohort Studies, Published And Unpublished Thesis And Literature. The Data Bases Searched Included Pubmed, Cochrane, LILACS, And Grey Literature Until 31/12/2017. Total Records Included 1139 Articles. The Participants Were At Least 15 Years Old, With And Pre And Post Cephalometric Radiographs Or CT Scans And Undergone First Premolar Extractions.

Results: Only Two Studies Included The Qualitative Synthesis. Conclusion: The Studies Showed The Orthodontic Extraction Changed The Hyoid Bone Position.

ABSTRACT
Dermoid Cysts Are Soft Tissue Cysts Of Embryonic Origin That Arise Along Lines Of Skin Fusion. They Formed Due To Abnormal Ectodermal Sequestration During Early Embryological Life. The Variable Types Of Craniofacial Dermoid Cysts May Be Related To The Initial Position Of Ectodermal Inclusion Within The Underlying Mesoderm. Variations In The Degree Of Ectodermal Inclusion Result In Differences In The Histology And Structure Of Dermoid Cysts At The Various Craniofacial Sites. Such Histologic Differences May Directly Influence The Severity Of Clinical Presentation. Five Cases Of Periorbital And Submental Dermoid Cysts Were Operated On Under General Anesthesia; All Cases Were Subjected To Immunohistochemical Staining With Cytokeratin, Vimentin And Chromogranin. There Was Significant Correlation Between The Expression Of Chromogranin In Periorbital And Submental Dermoid Cysts (P=0.007), While There Was Nonsignificant Correlation Between The Expression Of Cytokeratin In The Two Lesions (P= 0.08) And Also Nonsignificant Correlation Between The Expression Of Vimentin In The Two Lesions (P=0.43). Objective To Differentiate Histologically And By Using Immunohistochemitry Between Submental And Periorbital Dermoid Cysts.
