ABSTRACT


Purpose: This Study Was Performed To Investigate Incidence Of Bifid And Trifid Mandibular Canal In Egyptian Population Using Cone Beam Computed Tomography Materials &methods: A Sample Of Archived 600 CBCT Examinations Were Analyzed By Two Experienced Radiologist For The Presence Of Bifid Mandibular Canals, And Trifid Using On Demand 3d Software.

Results: Inter Observer Reliability Measured By Kappa Test Shows Good Inter Observer Agreement (kappa=0.788). Bifid Mandibular Canal Was Observed In 29.7% Of Cases And Trifid Mandibular Canal Was Observed In 2.3% Of Cases. Conclusion: Bifid Mandibular Canal Is Common In Egyptian Population And Present A Percentage Of 29.7% Of Cases In The Present Study And CBCT Examination Is Recommended To Detect This Important Anatomical Variation If Suspected Before Surgical Procedures.

EFFICACY OF TOPICAL APPLICATION OF PROPOLIS IN THE TREATMENT OF ACTIVE ORAL LICHEN PLANUS(RANDOMIZED CLINICAL STUDY)

ABSTRACT
Background: Oral Lichen Planus (OLP) Is A Chronic Inflammatory Disease Of Oral Mucosa, With Immunological Origin. Atrophic/erosive OLP Needs Appropriated Treatment, Due To Pain. So, The Aim Of This Study Is To Evaluate The Effect Of Topical Application Of Propolis Versus Topical Steroids For The Treatment Of Active Oral Lichen Planus.

Materials and Methods: A Double Blinded Comparative Study Was Conducted On 24 Patients. The Patients Were Divided Randomly Into Two Groups: Group I: Include 12 Patients Has Active OLP And Were Treated By Topical Corticosteroids Every 12 Hours For 2 Weeks. Group II: Include 12 Patients Has Active OLP And Were Treated By Topical Propolis 2.4% Every 12 Hours For 2 Weeks. All Participants In Both Groups Were Examined At Baseline (at The Start Of Treatment), After 2, 4 And 12 Weeks. During Each Appointment, Oral Examination Along With Photography (complete, Partial Or No Remission) Was Performed And The Patient?s Response Rates Were Determined Using The Visual Analog Scale (VAS), And Overall Clinical Improvement Determined By A Lesional Score (LS).

Results: The Clinical Observation (photography) Results Demonstrated Complete Remission In Corticosteroids (control) And Propolis (study) Groups (25% And 50% Respectively), Partial Remission 58.3% In Control Group And 50% In The Study Group While The Control Group Showed 16.7% Of Cases With No Remission. Within Subjects? Effect Of VAS Over Four Different Times Has Significant Difference (F (2.142, 66) = 84.48, P<0.001, ?2p =0.793) Which Means Significant Difference Of VAS Between Pre, 2, 4 And 12 Weeks After Treatment. The Interaction Of VAS And Group Over Times Is Insignificant Meaning That The Rate Of Change For VAS Over Time Between Those Taking Steroid And Those Taking Propolis Is Not Different (F (3, 66) = 1,673, P= 0.297, ?2p = 0.054). Between Subject Effect Shows That The Effect Of Group (steroid, Propolis) On VAS Over Times Is Insignificant (F (1, 22) = 0.610, P= 0.422, ?2p = 0.027). Within Subjects? Effect Of LS Over Four Different Times Has Significant Difference (F=1.514, 66) = 6.889, P0.006, ?2p =0.238) Which Means Significant Difference Of LS Between Pre, 2, 4 And 12 Weeks After Treatment. The Interaction Of LS And Group Over Times Is Insignificant Meaning That The Rate Of Change For LS Over Time Between Those Taking Steroid And Those Taking Propolis Is Not Different (F (1.514, 66) = 2.443, P= 0.156, ?2p = 0.085). Conclusion: This Study Supported That The Topical Use Of Propolis Has Proved To Be Beneficial, Safe And Effective In The Management Of Symptomatic Oral Lichen Plans.

THE EFFECT OF INDIRECT RESIN COMPOSITE MATERIAL THICKNESS ON BOND STRENGTH AND NANO-LEAKAGE OF CHEMICALLY-CURED UNIVERSAL ADHESIVE/DUAL-CURED RESIN CEMENT SYSTEM TO BOVINE DENTIN

ABSTRACT

Objectives: This Study Was Conducted To Evaluate The Effect Of Different Thicknesses Of Chair-side Resin Composite Inlay Material On Shear Bond Strength And Nano-leakage Of Chemically-cured Adhesive/dual-cured Resin Cement. Methods: For Shear Bond Strength Testing, 100 Bovine Permanent Anterior Teeth Were Used In This Study. Subsequent To Teeth Grinding And Formation Of Smear Layer. The Teeth Were Randomly Divided Into Four Experimental Groups (n= 25 Teeth/group). For Group 1, 2 And 3, The Resin Cement Was Light Cured Through 2 Mm, 4 Mm And 6 Mm Thick Resin Composite Cylinder Respectively. While Group 4, The Resin Cement Was Left To Cure Chemically With The Self-etching Adhesive (Co-cure). On Ground Dentin Surfaces, 100 Pre-cured Resin Composite Cylinders With Thicknesses Mentioned Above Were Cemented Using Chemically-cured Adhesive/dual-cured Resin Cement According To Manufacturers? Instructions. Specimens Were Stored In 100% Relative Humidity For 48 Hours. SBS Test Was Run At A Crosshead Speed Of 0.5 Mm/min Using A Universal Testing Machine. For Nano-leakage Testing, 12 Resin Composite Discs Bonded To 12 Bovine Teeth Were Assigned Into 4 Groups (n = 3) As For SBS Testing. Specimens Were Evaluated Using SEM Operated In The Backscattered Mode To Detect Dye Penetration. Statistical Analysis Was Performed Using One-Way ANOVA/ Turkey?s HSD Post Hoc Test.

Results: The Results Showed That The Co-cure Group Exhibited The Significant Lowest SBS Values Among All Tested Groups. The 2 Mm Thickness Revealed The Significant Lowest Nano-leakage Score (0%) Among All Tested Groups.

Conclusions: Light Curing Of The Dual Cured Resin Cement Was Crucial To Achieve High SBS Values. There Was A Positive Direct Correlation Between The Thickness Of The Resin Composite Material And The Degree Of Nano-leakage.

Background: Several Studies Have Shown That Chronic Hepatitis C (CHC) Infection Has A Negative Impact On Kidney Function, As Well As Survival Rate In Patients With Chronic Kidney Disease (CKD) Or On Hemodialysis.�Hepatitis C Virus (HCV)-related Liver Disease Is A Significant Cause Of Morbidity And Mortality In End-stage Renal Disease (ESRD) Patients Who Undergo Dialysis Or Kidney Transplantation (KT). Multiple Outbreak Investigations Which Evaluated Dialysis Patient HCV Risk Factors And Healthcare Exposures, Including Dialysis Treatment Factors, Identified Dialysis Facilities As The Most Likely Setting Where Transmission Occurred.

Aim: To Assess Frequency Of HCV Infected Patients Undergoing Hemodialysis. Methods: A Total Number Of 50 Patients Were Recruited From Cairo University Hospital In The First Week Of July 2017 And Both Demographic Data And Reported Data Of HCV Infection Were Identified.

Results: The Results Demonstrated That The Prevalence Of HCV Infection In Hemodialysis Patients Was 40%. Conclusion: End Stage Renal Disease Undergoing Dialysis Are More Prone To HCV Infection.

Introduction: Incorporation Of Antibacterials Into Restorative Materials, Has Evolved, Due To The Increased Understanding Of The Caries Process. However, The Impact Of This Mixture On The Properties Of These Materials Must Be Investigated. Up Till Now, There Are No In-vivo Studies Comparing The Survival Rate Between Conventional Glass Ionomer Cement [GIC] And Chlorhexidine Diacetate-modified GIC.

Purpose: Compare The Clinical Effectiveness Of Conventional Versus Chlorhexidine -containing Glass Ionomer Cement Using The Atraumatic Restorative Treatment Approach In Primary Molars. Methods: Forty-five Primary Molars, In 5-8-year-olds, With Single Surface Occlusal Caries Were Randomly Divided Into Three Groups. Group I: The Carious Primary Molar Was Restored With Atraumatic Restorative Treatment [ART] Approach Using Conventional GIC. Groups II And III: The Carious Primary Molar Was Restored With ART Approach Using Experimental GIC Containing Chlorhexidine Diacetate [CHX] 1% And 2%, Respectively (w/w). Clinical Performance For All Groups Was Assessed At 3, 6, And 12 Months. Results Were Statistically Analyzed Using Chi-square Tests.

Results: Results Showed No Significant Difference In The Survival Rate, Between All Groups, At The Different Time Periods. Conclusion: The Addition Of 1 And 2% CHX To GIC Did Not Compromise The Clinical Effectiveness Of GIC Used In The ART Approach.

Oral Squamous Cell Carcinoma (OSCC) Is A Characteristic Aggressive Tumor Representing A Significant Public Health Threat All Over The World. Oral Verrucous Carcinoma (OVC) Is An Uncommon Rare Low-grade Variant Of Well Differentiated OSCC, Which Accounts For 2-10% Of All Oral Carcinomas. Thrombospondin-1 (TSP-1) Is An Extracellular Matrix (ECM) Protein That Participates In Cellular Responses To Growth Factors, Cytokines And It Regulates Cell Proliferation, Migration And Apoptosis.

Aim: This Work Aimed To Assess The Relation Of TSP-1 Expression To Biological Behavior Of OSCC And OVC.

Materials and Methods: Forty Paraffin-embedded Specimens Divided Into 30 Cases Of OSCC And 10 Cases Of OVC Obtained From The Archives Of Oral Pathology Department, Faculty Of Dentistry, Tanta University.

ASSESSMENT OF MARGINAL ADAPTATION AND COLOR OF CAD-CAM-FABRICATED OCCLUSAL VENEER RESTORATIONS (IN-VIVO STUDY)

Statement Of Problem: Occlusal Veneers Are Introduced As A Restoration Of Erosive Cases. Evaluation Of Different Machinable Ceramic Material To Be Used For Occlusal Veneer Should Be Occured.

Purpose: To Evaluate The Marginal Adaptation And Color Of Occlusal Veneer Restorations Using Computer Aided Design And Computer Aided Manufacturer (CAD/CAM) Ceramic Material (IPS E.max CAD) And Hybrid Ceramic Material (VITA Enamic).

Materials and Methods: Ten Cases With Posterior Occlusal Surface Wear Were Selected And Divided Into Two Groups According To A Ceramic Material Which Was Used (5 Cases Each):- The First Group : IPS E.max CAD Ceramic Occlusal Veneers Cemented By RelyX Ultimate Clicker Resin Cement. (5 Cases). The Second Group: VITA ENAMMIC Hybrid Ceramic Occlusal Veneers Cemented By RelyX Ultimate Clicker Resin Cement. (5 Cases). Marginal Adaptation Was Measured Using Cone Beem Computed Tomography (CBCT) And Color Shade Was Meagured Using VITA Easy Shade Device After Cementation, 3,6,9 Months.

Results: Between The Two Materials There Was A Significant Difference Between The Two Materials. There Was A Significant Difference By Time Between Follow Up Visits But Still In Clinical Accepted Range. In The Two Materials There Was No Significant Difference Between Measurement Of Time Of Cementation And After 3 Months, There Is A Significant Difference Between (0 Day, 3 Months) And (6 Months, 9 Months), No Significant Between 6months And 9months. Conclusion: VITA Enamic Has Higher Marginal Adaptation Than IPS E.max CAD. IPS E.max CAD Has Color Stability More Than VITA Enamic. According To Color; VITA Enamic Can Be Accepted As Esthetic Restorartion In Posterior Region. Clinical Implication: Within The Limitation Of This Study; IPS E.max CAD And VITA Enamic Are Clinically Accepted To Be Used As Occlusal Veneer.

Objectives: The Present Study Was Designed To Assess The Effect Of CBCT With Different Voxel Sizes On The Accuracy Of Surgical Guide Fabrication On Computer Guided Implantology.

Materials and Methods: This Study Was Conducted On Ten Patients. The Patients Were Divided Into Two Groups. One Group Received Dental Implants Using Surgical Guides Designed From CBCT With 0.2 Mm Voxel Size While The Other Group Received Dental Implants Using Surgical Guides Designed From CBCT With 0.4 Mm Voxel Size. The Patients Were Asked For Post-operative CBCT To Compare The Accuracy The Surgical Guide. Linear And Angular Deviations Were Measured Between Planned And Actual Position In Both Groups. The Results Were Tabulated And Statistically Analyze.

Results: Surgical Guides Manufactured With CBCT Of 0.2 Mm Voxel Size Showed Statistically Significantly Lower Median Linear And Angular Deviation Than Surgical Guides Manufactured With CBCT Of 0.4 Mm Voxel In BL Direction. While There Was No Statistically Significantly Difference Between The Two Groups In Linear And Angular Deviation In MD Direction. Conclusion : There Was No Clinical Significantly Clinical Difference In Linear And Angular Accuracy Between Surgical Guides Manufactured From CBCT With Voxel Size Either 0.2mm Or 0.4 Mm.

DIODE LASER IN THE TREATMENT OF CHRONIC PERIODONTITIS IN DIABETIC PATIENTS

ABSTRACT
Background: Diabetes Mellitus (DM) Is One Of The Common Chronic Diseases In Adult Populations In The World. DM Has A Strong Influence On The Oral Cavity And Represents A Risk Factor For Gingivitis And Periodontitis. Conventional Periodontal Therapy Includes Both Non-surgical And Surgical Approaches. The First Category Includes SRP, The Adjunctive Use Of Chemotherapeutic Agent, Local Route Of Drug Delivery And Recently Laser Therapy Applying Lasers As An Adjunctive Or Alternative To Mechanical Treatment Had A Great Run In The Treatment Of Chronic Periodontitis (CP). Among Laser Applications, Low-level Laser Therapy (LLLT) Is Recommended For Its Pain-reducing, Wound-healing Promoter And Anti-inflammatory Effects. Objective: The Aim Of The Study Was To Assess The Effect Of Diode Laser As Adjunct To Scaling And Root Planing In Management Of Moderate Chronic Periodontitis In Diabetic Patients With Glycemic Control (Type ??-non Insulin Dependent).

Materials and Methods: Twenty Sites In Ten Diabetic Patients With Glycemic Control (Type ??-non Insulin Dependent) With Moderate Chronic Periodontitis Were Selected And Randomly Divided Into Two Groups. Group ? Ten Sites Received Only SRP; Group II Ten Sites Received SRP + LLLT. The Following Clinical Parameters (plaque Index (PI), Bleeding On Probing (BOP), Probing Pocket Depth (PPD), And Clinical Attachment Level (CAL) Were Recorded For Each Patient In Both Groups Before Treatment (baseline), 3 And 6 Months After Treatment. Gingival Crevicular Fluid (GCF) Samples Were Collected And Analyzed Using ELISA Test For Quantative Measurements Of MMP-9.

Results: The Results Showed Significant Improvement In The Mean PI, BOP, PPD, And CAL Gain For The Two Groups At 3 And 6 Months As Compared To Baseline Value. Furthermore, When Comparing The Two Groups, The Results Showed Non-significant Difference At All Study Period (P>0.05). While Group II Showed Better Reduction In The Mean PI As Compared To Group I At 3 And 6 Months For PI And At 3 Months For BOP At 6 Months. Moreover, Results Showed More Reduction In The Mean PPD And CAL At Group II Than Group I. Also, ELISA Test Demonstrated Significant Improvement In MMP-9 Levels Till 6 Month Regarding Both Groups. While There Were No Significant Difference When Comparing Group I And II At All Study Period (P>0.05). Conclusion: Both Treatment Modalities Resulted In A Significant Clinical Improvement Without A Clear Superiority Of One Procedure.

CLINICAL EVALUATION OF NON-INVASIVE MANAGEMENT OF EARLY OCCLUSAL DECAY

ABSTRACT


Aim: One-year Clinical Evaluation Of Different Non-invasive Management Of Initial Occlusal Carious Lesions.

Materials and Methods: Ten Patients With Four Initial Occlusal Carious Lesions Were Selected And Examined With DIAGNOcam And ICDAS Visual Criteria To Select Teeth Having ICDAS Code 1 And Code 2. Forty Lesions Were Divided Into Four Groups According To The Material Used For Treatment Were (group I: Icon?, Group II: FissealTM, Group III: Duraphat, Group IV: Left Without Treatment). The Teeth Were Subjected To Clinical Follow Up Schedule Representing Three Follow Up Periods (baseline, After 6 And 12 Months) During Which The Efficacy Of Tested Materials To Arrest Caries Progression Was Assessed Radiographically Through The Subtraction Of Pixel Grey Value Measurements. 1st Subtraction, Subtracting The Baseline Values From Those After 6 Months, 2nd Subtraction, Subtracting Values Of 6 From Those After 12 Months. The 3rd Subtraction, Subtracting The Baseline Values From Those After 12 Months.

Results: All The Tested Groups Showed Different Degree Of Continuous Lesion Progression With No Significant Difference In Group I, II And III. While, Group VI Recorded A Significant Difference With No Difference Between Its 1st Subtraction And 2nd Subtraction. However, There Was A High Significant Difference Between Its 3rd Subtraction Versus Both The 1st And 2nd Subtraction. The Effect Of All The Tested Groups On Caries Progression Showed A High Statistically Significant Difference At Each Evaluation Period With No Significant Difference Between Group I And Group II While Both Groups Were Significantly Different From Group III. However, Group IV Recorded A High Significant Difference From Groups I, II And III.

Conclusions: ICON?, FissealTM And Duraphat Were Effective To Hamper The Caries Progression Of Initial Occlusal Carious Lesions. The Initial Occlusal Carious Lesion Progressed Significantly In The Untreated Lesions Indicating That The Treatment Is Essential.
