THE EFFECT OF GENTAMYCIN ADMINISTRATION ON ORTHODONTIC TOOTH MOVEMENT IN A RABBIT MODEL

ABSTRACT
Objective: To Study The Effect Of Gentamycin (aminoglycoside) Antibiotic Administration On Orthodontic Tooth Movement.

Materials and Methods: The Study Was Conducted On Twenty Male Rabbits. They Were Divided Into Two Groups. Control Group In Which Only Orthodontic Tooth Movement (OTM) Was Conducted And The Gentamycin Group In Which Rabbits Were Given Gentamycin IM Injections In Regular Doses (3-5 Mg/kg/day) And OTM Was Conducted. Appliance Was Applied To The Lower Right Side. A Closed Coil Spring Was Connected Posteriorly To The First Premolar And Anteriorly To The Central Incisor. Animals Were Sacrificed After 21 Days. The Lower Right Halves Of The Mandible Of All The Animals Were Prepared For Specimen, Decalcified And Prepared For Histological Evaluation To Detect Changes In Osteoblastic And Osteoclastic Activity.

Results: There Were No Significant Changes In The Rate Of OTM Between Both Groups. Conclusion: Aminglycoside Antibiotics Are Safe To Be Administered During Orthodontic Treatment As They Do Not Alter The Bone Metabolism Yet Care Should Be Taken For Their Adverse Affect On The Kidneys.

USE OF PLATELET CONCENTRATES IN REGENERATION OF MECHANICALLY REMOVED CORONAL PULP IN MATURE TEETH, A COMPARATIVE ANIMAL STUDY

ABSTRACT
Objective: The Purpose Of This Study Was To Evaluate And Compare The Regenerative Potential Of Platelet Rich Fibrin And Plasma Rich In Growth Factors In Regeneration Of Mechanically Removed Coronal Pulp In Mature Teeth Histologically. Methods: Pulpotomy Was Performed In 24 Teeth From 2 Dogs, And The Pulp Chambers Were Filled With The Following Materials: (a) Groups 1: Platelet Rich Fibrin PRF; (b) Groups 2: Plasma Rich In Growth Factors PRGF. After 14 Days (groups 1/14-2/14) And 35 Days (groups 1/35-2/35), The Animals Were Euthanized And The Pieces Containing The Teeth Were Removed And Processed For Histological Analysis.

Results: Dentine Bridge Was Not Observed. The Radicular Pulp Response In Group 1/14 And Group 2/14 Consisted Generally Of Blood Vessels Dilatation, Diffusion Of The Platelet Concentrates Components, Areas Of Vacuolation Of The Odontoblastic Layer, Areas Of Pulp Tissue Disintegration And Fibrosis While In Group 1/35 And 2/35 These Findings Were Less Prominent Except For Fibrosis And Calcified Tissue Formation. Conclusion: The Observations Of This Study Might Help Develop A Predictable, Safe, And Cost-effective Vital Pulp Therapy Strategy, However Further Long Term Investigation And Research Is Needed To Reach More Conclusive Results About Their Possible Application In Human Patients.

ABSTRACT


Aim: The Objective Of This Study Is To Examine The Benefit Of An Intra-articular Platelet-rich Plasma (PRP) Injection And To Compare This With Arthrocentesis In Patients With Temporomandibular Disc Displacement With Reduction. Patients And Methodes: Twenty Four Patients Who Did Not Respond To Conservative Treatment Of Anterior Disc Displacement With Reduction Were Included In This Study. They Were Randomly Allocated To One Of Two Study Arms: The ?intervention? Group Who Treated With Intra Articular PRP Injection Or The Control Group Who Treated With Arthrocentesis.

Results: In This Study, Both Groups Showed Significant Improvement In TMJ Pain, Maximum Mouth Opening (MMO) And Clicking 2 Weeks After Treatment That Maintained Thereafter. Conclusion: It Is Possible To Conclude That Intra-articular PRP Injection Was Equivalent To Arthrocentesis Regarding Reduction Of TMJ Pain With More Beneficial Effects Of Arthrocentesis On MMO And TMJ Sound.

Aim: To Evaluate The Effect Of Arginine Versus Fluoride Containing Toothpaste On The Neutralization Of Microbial Acid Produced By Cariogenic Bacteria In Adult Population Using Chair Side Assessment Method.

Materials and Methods: A Total Number Of 14 Volunteer Participants Were Assigned Into This Study. Since It Is A Linear Cross Over Trial, All Subjects Were Exposed To Two Types Of Toothpaste In Two Phases. The Salivary PH Was Assessed According To Time In Relation To The Brushing With Toothpaste Before Brushing With Toothpaste And 10 Minutes After Brushing With Toothpaste.

Results: The PH Results Of The Current Investigation Revealed That For Resting PH (Baseline); Fluoride Toothpaste (5.49�0.83) And Arginine Toothpaste (5.54�0.85), Results Showed Insignificance Differences At P=0.876. The Same Applied For Terminal PH, Fluoride Toothpaste (5.69�0.76) And Arginine Toothpaste (5.94�0.79), Where Results Showed Insignificance Differences At P=0.389. Conclusion: Fluoride Containing Toothpaste As A Gold Standered Has The Ability To Neutralize The Acidic PH Of Saliva In High Caries Risk Patients And Arginine Containing Toothpaste Provided Similar Buffering Capacity As Fluoride Containing Toothpaste. However, The Immediate Efficacy Of Arginine Solely In Arginine Containing Toothpaste Was Not Revealed In This Study Over Fluoride Containing Toothpaste.

Randomized Clinical Trial Was Carried Outto Compare Between Chewing Efficiency Of Mandibular Complete Denture Constructed From Two Different Materials; Metal Reinforced Polyamide And Conventional PMMA. Twenty Four Completely Edentulous Patients. The Patients Were Divided Into Two Equal Groups, Group I Received Conventional Maxillary And Mandibular Heat Cured PPMA And Group II Received Conventional Heat Cured Maxillary Denture And Metal Reinforced Polyamide Mandibular Denture. Patients Of Both Groups Were Evaluated By Chewing Efficiency Measurements At The Time Of Prosthesis Insertion As Well As Three And Five Weeks Later By Sieve Method. Objective: This Study Was Conducted To Evaluate The Clinical Performance Of Self-etch Adhesive With Chlorhexidine Versus That Of The Conventional Adhesive Without Chlorhexidine In Class II Cavities In High Caries Risk Patients.

Materials and Methods: 37 Patients Of High Caries Risk Were Randomly Selected Each Patient Received At Least Two Class II Restorations Randomly,one With A Conventional Adhesive System (Futurabond M+ (VOCO, Cuxhaven, Germany)) , And The Other With The Intervention Adhesive System With 0.2 % Chlorhexidine (Peak? Universal Bond (Ultradent Products Inc., South Jordan, Utah, USA)

Results: The Assessment In This Study Was Done By Two Methods Clinical And Radiographic Assessment. There Was No Statistical Significant Difference Between Control And Chlorhexidine Groups In Immediate, 6 Months And 12 Months Follow-up. Conclusion: Self-etch Adhesive Systems With Or Without Chlorhexidine Seem To Bond Effectively With Very Good Results Through The Follow Up Period.

Objectives: To Assess The Curative Capability Of Adipose Tissue Derived Mesenchymal Stem Cells ?AT-MSCs? Contrasted With Bone Marrow Derived Mesenchymal Stem Cells ?BM-MSCs? On Healing Of Surgically Formed Cleft Lip In Dogs.

Materials and Methods: Split Mouth Experimental Study Was Done On 12 Healthy And Vaccinated Mongrel Dogs, 12-18 Months And Weight Range Of 20kgs. Dogs Were Divided Into 2 Groups: In -group A, The Test Side, The Surgically Created Cleft Lip On The Right Side Of Maxilla Was Transplanted With ?AT-MSCs? And Scaffold As Well As Growth Factors. While, In The Control Side, The Surgically Created Cleft Lip On The Left Side Of Maxilla Was Filled With Scaffold And Growth Factors Only. In-group B, The Test Side, The Surgically Created Cleft Lip On The Right Side Of Maxilla Was Transplanted With ?BM-MSCs? And Scaffold As Well As Growth Factors. The Control Side, The Surgically Created Cleft Lip On The Left Side Of Maxilla Was Transplanted With Scaffold And Growth Factors Only. Resorbable Sutures Were Used To Suture The Flaps. The Collected Samples Were Evaluated Clinically After 1.5- And 3-months Following Scarification Of The Dogs. The Data Was Analysed Using T-test And Descriptive Procedure (p=0.05).

Results: ?AT-MSCs? As Well As ?BM-MSCs? Are A Valuable Source For Soft Tissue Healing. Both Secrete Healing Markers ?VEGF, Collagen, TGF, And ALP? That Paly Significant Role In Restoring Soft Tissue Integrity. ?AT-MSCs And BM-MSCs? Effects Were Documented Clinically And Immunohistochemically, As There Were Areas Of New Vascularization That Assure Great Potentiality Toward Healing. ?AT-MSCs? Or ?BM-MSCs? Increase Regeneration And Healing Of The Defects As Early As 1.5 And 3 Months. Conclusion: AT-MSCs And BM-MSCs Are Interesting Tools In Soft Tissue Healing. ?AT-MSCs? In Experimental Investigations Revealed Comparable Results To ?BM-MSCs?. Also, It Is Easier In Harvesting, Requires A Secure Method To Get Stem Cells As Well As Lesser Chance Of Infection, And Lower Cost.

Introduction: Approaches For The Management Of Dental Caries Have Changed Dramatically In Recent Years, Evolving From The Traditional, Largely Restorative Treatment Approach To A Preventive Approach, Non Invasion Or Minimal Invasion. Resin Infiltration Technique Is A Novel Technology That Seems To Bridge The Gap Between Noninvasive And Minimally Invasive Treatment Of Initial Dental Caries, Postponing As Long As Possible The Need For A Restoration.

Materials and Methods: Ten Patients Were Recruited Through The Faculty Of Dentistry Clinics At Minia University For This Clinical Study. Digital Photographs Were Obtained Under Standardized Conditions Using A Fixed Digital Camera With A Macro Lens And A Point Source Flash Positioned At A Fixed Focal Distance From The Patient In A Dark Room To Eliminate The Room Light Variant. White Spot Lesions (WSL) Were Resin Infiltrated With Icon (DMG, Hamburg, Germany). Remin Pro Was Applied Chair Side In The Clinic By The Operator With A Polishing Brush On A Low Speed Handpiece For 5 Minutes On The Enamel Surfaces. These Procedures Were Repeated For 7 Consecutive Days. Result: Resin Infiltration Showed Significant Color Improvement Than Remain Pro. Conclusion: Resin Infiltration Is A Novel Protocol For White Spot Lesions And It Is Recommend To Combine Resin Infiltration With Fluoride And Hydroxyapatite Applications To Achieve Ideal Masking, Treatment And Preventive Protocol.

EFFECT OF CELEBREX PRE-MEDICATION ON THE INTRA AND POST-TREATMENT ENDODONTIC PAIN IN TEETH WITH SYMPTOMATIC IRREVERSIBLE PULPITIS(DOUBLE BLIND RANDOMIZED CLINICAL TRIAL)

Introduction: Achieving Profound Pulpal Anesthesia In Teeth With Irreversible Pulpitis Is Quite Challenging And Sometimes Difficult. This Study Evaluated The Efficacy Of Selective COX-2 Inhibitors As Premedication In Controlling Intra-operative And Post-endodontic Pain After Single Visit Endodontic Treatment Of Mandibular Molar Teeth With Symptomatic Irreversible Pulpitis As Compared To Placebo. Methods: In A Randomized Double-blinded Clinical Trial, 46 Patients (23 Per Group) With Irreversible Pulpitis Were Given 200mg Celebrex, Or Placebo 1 Hour Before Local Anesthesia. Each Patient Recorded Their Pain Score On A Visual Analog Scale Before Taking The Medication, During Access Cavity Preparation And During Root Canal Instrumentation. No Or Mild Pain At Any Stage Was Considered A Success. Data Were Analyzed By The Mann-Whitney U Test For Comparison Between The Two Groups And Analysis Of Variance Tests.

Results: There Was A Statistical Gradual Reduction Of Pain In Both Groups Through All Time Intervals. At 12 Hours Post-operatively, Celebrex Group Showed Statistically Significant Higher Percentage Of Pain Reduction As Compared To The Placebo Group (p< 0.05). Regarding Long Buccal Infiltration And Intra-pulpal Anesthesia, Celebrex Group Showed Significantly Lower Percentage Of Using Supplemental Anesthesia (p=0.008).

Conclusions: Premedication With 200mg Single Dose Celebrex Prior To Initiation Of Endodontic Treatment Augmented The Effect Of Inferior Alveolar Nerve Block And Has A Satisfactory Effect On Reduction Of Post-operative Pain.

Objective: The Purpose Of This Study Was To Evaluate The Shaping Ability Of S5 Rotary System In Relation To ProTaper Universal System In Preparation Moderately Curved Root Canal Their Effect On The Canal Transportation, Centering Ability And Canal Curvature Change.

Materials and Methods: Collected Thirty Mesiobuccal Root Canals Of Extracted Human Mandibular First Molar Were Divided Into Two Equal Groups; ProTaper And S5. The Samples Were Mounted In Two Moulds Of Acrylic Resin For Pre-instrumentation Imaging Using Cone Beam Computed Tomography (CBCT). After Root Canal Instrumentation, Post-instrumentation Imaging Using CBCT Was Done With The Exacted Parameters Of The Pre-instrumentation Imaging. The Pre And Post-instrumentation CBCT Images Were Superimposed To Measure The Dentin Thickness At Three Root Canal Levels; Canal Transportation And Centering Ability Were Recorded According To Formula Introduced By Gambill Et Al (1996), While The Change In The Angle Of Canal Curvature Was Recorded Following Estrela Et Al (2008). Statistical Analysis: Data Was Tabulated And Statistically Analyzed; Independent Samples T-test Was Used For Comparing The Variables Between The Two Groups At Each Root Canal Level And Also Was Used For Comparing The Percentage Of Change In The Canal Curvature Angle Between The Two Groups. The Significance Level Is Considered At P � 0.05.

Results: There Was No Statistically Significant Difference Between The Two Rotary Systems In Canal Transportation Except At The Coronal Level; S5 Recorded Statistically Lower Mean Value Of Root Canal Transportation Than ProTaper System. The Centering Ratios And The Changes In The Angles Of Canal Curvature After The Use Of Both Systems Were Not Statistically Significant. None Of The Used Systems Showed Perfect Centering Ability. Conclusion: Under Limitations Of This In Vitro Study, It Can Be Concluded That; Preparation Of Moderately Curved Mesio-buccal Root Canals With S5 Was Associated With Lesser Coronal Transportation And Similar Middle And Apical Transportation, Centering Ability, And Canal Curvature Change When Compared To ProTaper.
